We are wri Ɵ ng to let you know that the Chabot-Las Positas Community College District Board of Trustees passed a mandate that all students a Ʃ ending classes, programs, using services or par Ɵ cipa Ɵ ng in any ac Ɵ vity on any campus or otherwise in person must be fully vaccinated against COVID-19. Also, please remember that there is s Ɵ ll a mask mandate in place when indoors on any campus or in any district facility. Students who are taking in-person classes or who need to come to campus for any reason (to access tutoring, student services, the Library, STEM Center, CTE, etc.) are required to submit proof of vaccina Ɵ on via CLEARED4. Students must have their vaccina Ɵ on informa Ɵ on or exemp Ɵ on approved before becoming eligible to register for in-person classes. Registra Ɵ on for Spring 2022 classes begins on November 9, 2021. Medical or religious exemp Ɵ on forms are available from the President’s O ffi ce at each college. Students a Ʃ ending online-only classes for the Spring 2022 semester and who will not be visi Ɵ ng campus for any reason, are not required to upload their vaccina Ɵ on informa Ɵ on. Please do not delay ge ƫ ng vaccinated as the college(s) may need addi Ɵ onal Ɵ me to process and verify vaccina Ɵ on status. Prior to your registra Ɵ on appointment date, you will receive an email from CLEARED4 with direc Ɵ ons on how to upload your vaccine veri fi ca Ɵ on. Three op Ɵ ons will be o ff ered for submi ƫ ng proof of vaccina Ɵ on: • Op Ɵ on 1: Upload a photo or scanned copy of completed CDC COVID-19 vaccina Ɵ on card for either Johnson & Johnson, Moderna or P fi zer vaccina Ɵ ons. • Op Ɵ on 2: Upload your digital vaccina Ɵ on record (typically a JPG or PNG fi le), which you can obtain at myvaccinerecord.cdph.ca.gov . • Op Ɵ on 3: Connect health data through CLEARED4 to your Electronic Health Record (EHR) provided by your insurance and/ or health care provider. If you are vaccinated, please submit your vaccina Ɵ on to the CLEARED4 link you will receive shortly. If you are not ge ƫ ng vaccinated before your registra Ɵ on opens, you will not be allowed to register for in-person classes or allowed on-site access to any college campus or district facility. If you are approved for an exemp Ɵ on from vaccina Ɵ on, you will be required to wear a fi Ʃ ed N-95 mask while on-site at any college campus or district facility. You will also be required to provide a weekly nega Ɵ ve COVID-19 test result.
